A simple question, rarely answered clearly: 15 to 20 minutes, then at least 20 minutes off before applying again.
Here is why, and when you should stop sooner.
Why 15 to 20 minutes
It is the duration commonly used in practice and the one applied in the available studies on cold and head pain. In the 2013 randomised trial on neck cooling, maximum pain reduction was measured at 30 minutes of application. An earlier pilot study used sessions of 25 minutes.
Beyond that, the benefit stops increasing while the risk to the skin rises. Under 10 minutes, the effect is usually too slight to notice.
The barrier rule
Never place ice or frozen gel directly on the skin. A cold burn can appear within minutes, and facial skin is particularly vulnerable.
A thin layer of fabric between the cold source and the skin is enough. Masks designed for this use already include that layer — it is their main advantage over a bare ice pack.
Stop immediately if
- The sensation becomes painful rather than soothing
- The skin turns very pale, white or blotchy
- You feel numbness that persists
- Dizziness or nausea appears — cold intolerance is real and has been reported in studies
When to seek medical advice first
Applying cold is not harmless for everyone. Ask a healthcare professional if any of the following applies to you:
- Any loss of sensation in the face, whatever the cause — you will not feel the warning signal
- A known circulatory disorder, particularly Raynaud's syndrome
- Diabetes with nerve involvement
- Broken, irritated or recently operated skin in the area
- Cold urticaria or a known hypersensitivity
How many times a day
There is no strict established limit, but a sensible guide is to leave at least 20 minutes between applications, so the skin returns to its normal temperature.
If you find yourself needing cold several times a day, every day, the question is no longer how long to apply it: it is how often you are getting headaches. That is a reason to see a doctor.
Fridge or freezer?
For a gel mask, around two hours in the fridge gives a mild, long-lasting coolness that the face tolerates well.
The freezer gives a sharper, shorter cold and needs a few minutes at room temperature before use. For the eye area, the fridge is generally more comfortable.
